Do I need a referral?
It depends on your insurance plan. Call us and we'll check.
Do you do physical therapy and chiropractic care?
Yes. Dr. Kaczmarek is licensed as both a physical therapist and a chiropractor, so both can be part of the same visit.
What should I bring to my first visit?
Your insurance card, a photo ID, and any imaging reports. After surgery, also bring your surgeon's instructions.
What should I wear?
Comfortable clothes you can move in, that make it easy to reach the area being treated.
Who will I see?
For physical therapy and chiropractic care at Union Square Rehab & Health, you'll see Dr. Kaczmarek, one-on-one, every visit. Also at our location are Erica Kaiser, LMT, for massage therapy (23 years in practice) and Dr. Thomas Sickler, DC, at Sickler Spine & Soft Tissue (11 years in practice). Like us, both focus on hands-on, one-on-one care.
